When you register as a client of LJD Physiotherapy, you provide consent for us to access and use your personal information so we can provide you with the best possible care. Only staff who need to see your personal information will have access to it. If we need to use your information for anything else, we will seek additional consent from you to do this.

In order to provide a safe and effective Physiotherapy service to you we collect some of your personal information. We also use it for directly related business activities, such as financial claims and payments, audits, and staff training. For these purposes LJD Physiotherapy is considered the “controller” of the personal information.

Under the UK General Data Protection Regulation (UK GDPR), the lawful bases we rely on for processing this information is that we have a legitimate interest.

The type of personal information we collect:

The information we collect about you is:

  • Personal identifiers, contacts, and characteristics (for example, name, date of birth, age, contact details, address, email address, telephone numbers).
  • Health related information (for example, past medical history, medications, reason for having physiotherapy, allergies, social history, your GP practice).
  • Occasionally we collect health care identifiers (such as your NHS number) in order for us to communicate to NHS providers (such as your GP) with your consent.
  • In some circumstances we will collect information from you about your private healthcare insurance provider (if you are using one).
  • Information about your home (such as, the layout, number of rooms, size of rooms, ownership) to help to determine what equipment would be appropriate to provide.
  • Information on accessing your residency (such as, key safe numbers, gate codes, codes to access supported living environments) to safely be able to access the residence of clients who are unable to answer the door themselves or in emergency situations.
  • Next of Kin information when provided by you (such as, name, address, telephone number).
  • Details required for you to pay and record input from our service (such as, invoices and receipts).

How we get the personal information and why we have it

Most of the personal information we process is provided to us directly by you for one of the following reasons:

  • Providing you with a safe Physiotherapy service. We use information about your past medical history and current health problems to determine which treatments are safe to use.
  • Helping us to perform daily business activities, such as planning journeys, providing invoices, sending referrals to other healthcare providers and ordering of equipment for delivery.
  • To understand our business better to enable us to target certain populations with future business advertising (for example, certain age groups).
  • To audit our business to ensure we are meeting professional body requirements (such as standards outlined by the Health and Care Professions Council).
  • For staff training and learning purposes for staff to ensure maintenance and encourage improvements in standard LJD Physiotherapy service provision.

We may also receive personal information indirectly, from the following sources in the following scenarios:

  • Users of our website ljdphysiotherapy.co.uk who complete an enquiry will provide their name, email address, phone number and potentially some health related information. By using the website they give their consent to this policy. This can be withdrawn at any time in writing by email (ljdphysiotherapy@proton.me).
  • Referrals from other sources such as, General Practitioners, other private health care services, Health Insurance companies.

We may share this information for:

  • Other healthcare providers such as your GP practice.
  • Your private health insurance company.
  • Your next of kin or relevant person as identified by you (with your consent).
  • When it is required or authorised by law (eg court subpoenas).
  • To third party organisations (with your consent) so that they can provide you a service (such as an equipment delivery company like Nottingham Rehab Supplies).
  • When it is necessary to lessen or prevent a serious threat to a patient’s life, health or safety or public health or safety, or it is impractical to obtain the patient’s consent.

At LJD Physiotherapy only people who need to access your personal information will do so. Mostly, this will be those directly involved in your care. Other than for the aforementioned reasons, LJD Physiotherapy will not share your personal information with any third parties without first gaining your consent. LJD Physiotherapy will not share your personal information with anyone outside of the UK unless consented to do so by you.

How we store your personal information

Your information is securely stored:

  • Electronic notes will be stored on an encrypted hard drive on password protected devices.
  • Any mobile phones or laptops will be password protected and locked away when not in use.
  • Any personal information or invoices shared by email are encrypted and access to the emails is password protected.
  • Anti-virus software is installed on LJD Physiotherapy computers/laptops.
  • Any electronic device used by LJD Physiotherapy to be disposed has a factory reset performed and is disposed of in a safe manner.
  • Any hard copies of personal information is stored in a locked filing cabinet within a locked building.
  • Any hard copies of personal information to be destroyed is shredded on-site.
  • LJD Physiotherapy staff access personal information on a strict “need to know” basis avoiding any unnecessary use of personal data.

We are legally obliged to keep all healthcare related personal information for 8 years or until Children reach 21. We will store any administrative data for 6 years in case this is required by HMRC. Any information not required to be kept for these time periods will be destroyed as soon as possible.  We will then dispose your information by:

  • wiping any electronic records.
  • Shredding and incinerating any paper records.

LJD Physiotherapy reserves the rights to make any changes and updates to this privacy policy as and when needed without giving notice and our most up-to-date version will be found on our website www.ljdphysiotherapy.co.uk.

Your data protection rights

Your right to rectification – You have the right to ask us to rectify personal information you think is inaccurate. You also have the right to ask us to complete information you think is incomplete.

You are not required to pay any charge for exercising your rights. If you make a request, we have one month to respond to you.
